allow you to help many of your patients receive the dental care they need and at the same time

s monthly dental premiums are continuing to go up, while at the same time the dental benefits are
continuing to go down.  Many of your patients are without any dental benefit coverage and many are
unhappy with their present dental plans.  Patients do not like to choose unknown doctors on limited
insurance lists of providers.  These restrictions limit their freedom to choose the doctors of their
choice.  Patients want to go to doctors that they trust and feel confident with.

having to wait months for the insurance companies to respond. It means your patient receives the
dental treatment without having to wait for “special approval.”  It means the dental work the patient
desires is between the doctor and the patient.  There is no more “non-doctor” insurance claims
adjuster deciding what type of dentistry your patient can and can’t have done.

It means no more pre-determination forms to file to insurance companies and waiting 90 to 120
days for a response.  The response is usually your pre-determination has been lost and a new one
needs to be resubmitted.  It means your patient is in complete control of their own dental health

The AFFDENT Program

This is a one-year contract patient membership discount plan.  It automatically renews each year.  
Think of joining a health club or golf country club.  These members pay a monthly membership fee
to partake of and participate with all the various amenities.  Likewise, your patients pay a monthly
membership fee in return for discounted dental services from your private dental fee schedule.

There is a singles plan, a couples plan and a family plan.  A yearly benefit dollar amount maximum
is allowed for each individual within each plan.  A monthly membership is automatically withdrawn
from the patient’s bank account and deposited into a central AFFDENT banking hub.  Each month a
small accounting and administrative fee is taken off each contract and the bulk of the monthly
membership fee is automatically deposited back into the participating doctor’s business account.

In today’s world, there is no such thing as patient loyalty.  Most doctors have encountered the
situation where they have bent over backwards taking care of a patient and their family’s dental

Many patients are denied by a third party financing company because in years past they hit a
financial speed bump and have a fair to poor credit rating due to a past divorce, financial crisis, etc.  
Many of these patients are trying to financially get back on track.  This plan allows many of them to
get some dental work done.  These patients still need to be eligible to hold a current credit card in
order to sign up for these plans.  The credit card on file at AFFDENT insures the doctor gets paid if
there are insufficient funds in the patient’s bank account during the automatic monthly withdrawals.

Your office will administer the plans directly to your patients.  There is no need for your patients to
contact a third party company for information.  All questions regarding AFFDENT plans can be
answered by your office administrators.

AFFDENT Plan Specifics

There are three plans to choose from: a singles plan, a couples plan and a family plan.  The
monthly membership fees are as follows:

Singles Plan:  $41/month
Couples Plan: $61/month
Family Plan:   $81/month

The yearly maximum benefit is a total of $1,250 for each individual within each plan.  Example: With
a couples plan, each person will have a yearly $1,250 benefit maximum.  With a family of four, each
individual will have a yearly $1,250 benefit maximum.


AFFDENT Type I Dental Services

The patient has no out-of-pocket expenses for Type I dental services.  The AFFDENT program
allows 100% discount for all consultations, complete exams, periodic exams, all types of
radiographs, sealants, child fluoride treatments and up to two basic prophylaxis per year.  All Type I
services are applied towards the patient’s $1,250 yearly maximum benefit.  Meaning, any fee the
patient does not pay out of pocket, is deducted from the $1,250 yearly maximum.

AFFDENT Type II Dental Services

Type II dental procedures include: any type of composite or amalgam fillings, simple extractions,
periodontal SRP therapy, periodontal gross debridement therapy, basic root canal therapy, direct
pulp caps, post and core buildups, soft tissue crown lengthening, any type of PFM crowns and PFM
bridges, any type of inlays or onlays, dentures, partials, any type of denture or partial repair, any type
of soft or hard denture relines and all types of transitional partial dentures.

There will be a 50% out-of-pocket fee due the day of service.  The 50% discounted portion is
applied towards the patient’s $1,250 yearly maximum benefit.  Any treatment plan that exceeds the
$1,250 yearly maximum benefit including all the applied Type I services, is the patient’s
responsibility to pay 100% of the doctor’s private fees.

With any treatment exceeding the yearly maximum benefit, the patient may choose one of your
current payment options offered to your fee-for-service patients such as cash, check, credit card or
one of the many types of third party outside financing companies.
